I shall not make any false statements or submit any document
which I know to be false.

I shall work only for the employer and in the occupation
specified in the work permit or visit pass.
|

I shall not work for any other person, in any capacity, with
or without monetary payments.
|
| 4 |
I shall not engage in or participate in any business.
|
| 5 |
I shall reside at my employer's residential address as
stated in the work permit. (applicable to domestic
workers from non-traditional sources).

| 6 |
I shall carry my original Work Permit or visit pass with me
at all times and must produce it for inspection on demand by
any public officer.
|
| 7 |
I shall submit myself to and pass a medical examination by a
registered Singapore doctor as and when directed by the
Controller of Work Permits.
|
| 8 |
I shall not go through any form of marriage or apply to
marry under any law, religion, custom or usage with a
Singapore Citizen or Permanent Resident in or outside
Singapore without the prior approval of the Controller of
Work Permits while I hold a work permit and also after my
work permit has been cancelled. I will be expelled and
prohibited from entering Singapore if I breach this
condition.

I shall not cohabit with a Singapore
Citizen or Permanent Resident. |
| 10 |
I shall not become pregnant or deliver any child in Singapore
during the validity of my work permit or visit pass.

| 11 |
I shall not engage in any relationship with a Singapore
Citizen or Permanent Resident that will result in the birth
of any child.
|
| 12 |
I shall not indulge or be involved in any illegal, immoral
or undesirable activities including breaking up families in
Singapore.
|
| 13 |
Conditions 8, 9, 10 and 11 shall continue to apply to me
even after I cease to hold a work permit. Breach of these
conditions shall result in my being expelled and prohibited
from entering Singapore.

I shall report to the Controller of
Work Permits as and when I am required by the Controller to do
so. |

I understand that if I breach any of
the above mentioned conditions, my work permit or visit pass will be revoked and I can be prosecuted in Court
